Safeguarding concerns which include physical signs of harm or physical injury should include a clear and detailed written description of any injuries that are seen directly by staff member reporting the concern. Therefore, in relation to Staffordshire referrals only, Annex D (Body Maps) of the Safeguarding and Child Protection Policy can be disregarded.
